LES SCHWAB PHOTO RELEASE

By making this submission to the Les Schwab website, I hereby grant to Les Schwab Warehouse Center, Inc. and its affiliates, subsidiaries, licensees, successors, assigns and those acting with its authority (“Les Schwab”), the unrestricted absolute, perpetual, worldwide right to reproduce, copy, exhibit, display, perform, transmit, broadcast, distribute, modify, create derivatives, and otherwise use my photograph(s), name and likeness (“the photograph(s)”) on or in connection with Les Schwab’s marketing, advertising or other promotional activities. This grant includes, without limitation, the right to use the photographs(s) or any part thereof in combination with or as a composite of other matter, including, but not limited to, text, data, images, photographs, illustrations, animation and graphics, video or audio segments of any nature, in any media or embodiment, now known or hereafter developed, including, without limitation, print, film, videotape, DVD, broadcast, digital transmission and electronic/online media. This grant further includes the right to use my name, whether in original or modified form, or a fictitious name, in connection with the photograph(s).

I hereby agree to release and discharge Les Schwab and its employees, officers, directors, agents, licensees, successors and assigns from any and all claims, demands or causes of action that I may now have or may hereafter have for libel, defamation, invasion of privacy or right of publicity, infringement of copyright or violation of any other right arising out of or relating to any utilization of the photograph(s) or my name, including, without limitation, inadvertent errors, such as blurring, distortion or alteration, or based upon any failure or omission to make use thereof.

I understand that Les Schwab is relying on my consent to use the photograph(s) in its promotion of various products, and I acknowledge that I am to receive no compensation with respect to any matter referred to herein. All images, including but not limited to electronic or non-electronic negatives, positives and prints, are and will be owned by Les Schwab. Any and all of the rights granted herein will be freely assignable by Les Schwab. I acknowledge that in no event will I have the right to enjoin the distribution or exploitation of the photograph(s) in connection with Les Schwab’s promotional activities. I hereby relinquish any right that I may have to examine or approve the completed product(s) or advertising copy or printed matter that may be used by Les Schwab.
